Request your appointment
Choose your service and a preferred date and time online. We review every request and confirm your appointment with you. Don’t see a slot that works? Text or call (818) 814-9116 and we’ll try to fit you in.
Complete your forms and payment
Once your appointment is confirmed, you’ll receive an email with secure links to complete your intake forms, questionnaires, consent forms, and visit payment.
Meet with your provider
Connect through a secure video appointment or meet with your provider in person, depending on the service scheduled.
Leave with a clear plan
Receive a personalized care plan, which may include prescriptions, laboratory or imaging orders, and follow-up care when medically appropriate.
Already a patient?
Schedule a follow-up, complete forms, securely message your provider and review available health information through your patient portal.
